Thank you for choosing Gladly Music for Music Therapy. To get started smoothly, please follow these steps in order.

Step 1: Complete the Intake Form

Please take a few minutes to fill out the Intake Form. This helps me understand your needs and get everything ready for you.

Step 2: Review & Sign the Service Agreement and Optional Media Release Form

Once I receive your completed intake form, I will email you the Service Agreement and optional media release form using the email provided in your intake form.

Step 3 (optional): Using Step Up for Students Scholarship or CMS Insurance

If you plan to use a scholarship (such as Step Up for Students) or bill through insurance (CMS only), let’s get that set up. I’ll guide you through the process to make sure everything is in place before your first session.

Step 4: Submit Credit Card Authorization

Once you’ve signed the contract, please submit your credit card authorization here to keep a card on file for payments and cancellation fees.

Step 5: Establish a recurring session time.

Here’s how it works:

  1. I will email or text you 2-3 session time options that align with your availability indicated on the intake form.

  2. Let me know which one works best for you.

  3. Once we confirm, I’ll send a Google Calendar Invite with your session details

  4. The invite will repeat weekly, and you’ll get automatic reminders from your calendar

    Please be sure to click “Yes” to accept the calendar invite
    This ensures the session is added to your calendar and you'll receive automatic reminders.

    Need to cancel or reschedule later?
    You can decline the invite or propose a new time directly in your Google Calendar. I’ll get notified and we’ll work out a new time together.
